1) Patients residing in the states of Florida and Georgia will have care rendered via Resurgens Medical LLC.

2) Patients residing in the state of New Jersey will have care rendered through Simonetti Medical PLLC.

3) The Resurgens Medical brand and trademark rights are held by Resurgens Medical Solutions, a Delaware LLC. They are currently licensed to be used by Resurgens MSO LLC (Delaware), and Resurgens Medical LLC, a licensed Florida Healthcare Clinic. 

4) Compounded medication laws vary from state to state, and federal law is evolving rapidly. Not all compounded medications are available in all 50 states, and as regulations and manufacturer backlogs change some medications may be discontinued. Should your prescribed medicine no longer be compounded Resurgens Medical will work with you to find a commercial equivalent, or if there is none a reasonable alternative.
